It states that his cause of death was “sequelae of blunt traumatic injuries” resulting from a “ground level fall,” with “arteriosclerotic cardiovascular disease” listed as a “contributing condition.” It is mentioned that Simmons had experienced an episode of dizziness and had collapsed prior to his death. Toxicology also found the presence of certain medications but concluded they did not contribute.
